Dear friends! Here’s a list of documents needed for the Commission DOH in Ukraine to consider referral to treatment abroad
Sending the citizens of Ukraine (of any categories of patients without differentiation for children, adults, military) for treatment abroad is subject to the requirements of the procedure for sending citizens abroad for treatment, approved by the Cabinet of Ministers of Ukraine of 08.12.1995 number 991, and the DOH order of 17.07.2014 number 502 “On ensuring the organization of citizens referrals for treatment abroad.”
The documents:
1. Personal statement of patient (parents).
2. Letter of referral by regional health administration.
3. Extract from medical history.
4. Conclusion of the main free lance specialist of DOH about the need for referral to treatment abroad with substantiation of reasons for referral and guidance of recommended specific foreign clinic.
5. Invitation of the clinic indicating the cost of treatment and clinic bank account (with translation into Ukrainian).
6. Payment document on the cost of treatment (Invoice).
7. A written agreement with the clinic.
8. Certificate of residence and family composition (Form 3).
9. Information on salary or pension for the year.
